On Monday, August 3rd, PHS fall sports will resume activities in Phase 2 of the Re-Entry plan with some important modifications (the Re-entry form can be found on the GoPortageIndians home page along with Emergency Consent and Concussion forms):
- Face masks/coverings are MANDATORY when not involved in rigorous activity. (Examples: getting out of your vehicle – MASK ON; walking to your practice area – MASK ON; stretching – MASK ON; running – MASK OFF; rigorous sport-specific drill work – MASK OFF; coach addressing the team -MASK ON; leaving practice – MASK ON; going to the restroom – MASK ON)
- If anyone in your household is awaiting the results of a COVID test, you are NOT permitted to attend any practice sessions until those results have been returned and are negative.
- Daily Health Screen Questionnaires and Temperature Checks will continue. Contact your sport coach if you do not have the link to the Daily Health Screen.
Reminder: students must have all required paperwork on file in the athletic office in order to participate (Note: if you intend to use last year’s physical for this school year, you must have turned in the IHSAA 2020-2021 Health History Update Questionnaire And Consent & Release Certificate.)
